CardioRenal Vision is a Physician owned and led national company focused on providing skilled nursing communities and post-acute care providers with direct access to proactive cardiac, renal and pulmonary services, including dialysis, telemedicine and remote patient monitoring. By practicing a preventive rather than reactive approach, CardioRenal Vision has a proven model of reducing hospitalizations and improving outcomes for their patients.

We aim to reduce hospitalizations and improve patient outcomes by treating patients in their current setting. Transitional Post-Acute Specialty Care
CRV has perfected transitional specialty care. Our Transitional care clinical model bridges the gap between the hospital discharge to post-acute facility and to the patient’s home. We discuss the patient’s care with the hospital specialist and outpatient specialist to ensure all their needs are met.
House Calls - ALF/IND Cardiology
Pioneered by Dr. Junaid Ahmed, CRV provides cardiology services in assisted living and independent living facilities, allowing for a less stressful and more convenient appointment within your own home.
Sports Cardiology
We recognize the importance of athletics for cardiac patients and the significance of the sudden cardiac deaths that have occurred in a small number of athletes. Sports Cardiology is a relatively new area of medicine focused on the diagnosis and treatment of cardiac conditions in athletes.
